DSH plugin: closes ai-memory's consolidation loop. When a DSH session ends it runs ai-memory finalize-session, so captured observations become searchable wiki pages every other harness ai-memory serves can read. Ships the hand-written DSH capture-hook template and diagnoses a missing ai-memory or unwired hooks at startup.
